Screening form
Message to Patients

5/1/2020
Dear Patients and friends,

We hope this letter finds you and your family in good health. Our community has experienced difficulties over the last few months, and we are looking forward to resuming normal habits and routines. While many things have changed, one thing has remained the same: our commitment to your safety.

Infection control has always been a top priority for our practice. Our protocols are established to ensure that treatment is both safe and comfortable.

Our office follows infection control recommendations made by the American Dental Association (ADA), the CDC, and OSHA maintain compliance with any new rulings or guidance issued.

In order to protect our patients and staff, the following changes to our office protocol will be implemented:

-
Appointments will be managed to allow for social distancing between patients. We request that you come alone unless you require assistance (ie. children, disabled or elderly patients).
-
Multiple people from the same household may come together if they are all receiving care.
-
All people entering will have to go through the proper screening to be present in the office and need to wear a mask while in the office except at the time dental treatment is being provided. Due to the current shortage of protective wear, we request that you come with your own mask.
-
Prior to your appointment, office staff will ask a series of screening questions over the phone.
-
Upon arrival to your appointment, you will be presented with a screening form with the similar questions. We strongly encourage you to fill out the form before coming. (Please click on the word file in the upper right corner of this page, print, and complete.) These forms will also be provided on the outside of the front door on a clipboard, although completing the form ahead of time will allow us to begin your visit sooner.
-
We request you apply the hand sanitizer available in the lobby upon entering the office.
-
Your temperature will be taken in the reception area with a touchless thermometer.
-
The waiting room will no longer offer magazines or children’s books as these items are difficult to clean and disinfect.

We will do our best to allow greater time between patients to reduce waiting times for you, as well as to reduce the number of patients in the reception area at any one time.

We look forward to seeing you again and are happy to answer any questions you may have about the steps we take to keep you, and every patient, safe in our practice. To make an appointment, please call our office at (713)-621-7616 or visit our website at www.redmansmiles.com

Thank you for being our patient. We reassure you that these unique conditions are not permanent. We value your trust and loyalty and look forward to welcoming back our patients, neighbors, and friends

Sincerely,
Darren Redman DDS, Marcy, Lauren, Marge, and Stephanie
